Horizon Health is a Critical Access, Rural Health Facility comprised of 25-inpatient beds located in Paris, IL & a multitude of outpatient clinic settings including Family Practice and Specialty Clinics in Paris and surrounding cities. Position Summary:
The HHA is responsible for providing direct and indirect patient care under the direction of a RN in the patient's home. Provides individualized attention, which encourages each patient's ability to maintain or attain the highest practical physical, mental, and psychosocial well-being.
Essential Functions:
- Assists patients with activities of daily living, providing for personal care and comfort, assists in the maintenance of a safe and clean environment for an assigned group of patients and performs more complex clinical skills under the direction of an RN / provider
- Partners with individuals and families to assist them in making progress toward self-care
- Promotes family members to share responsibilities of care and encourages the patient to participate in the life of the family, when possible
- Provides patient care with consideration of the patient's needs, age, culture, and appropriate standards of care
- Involves patient/family in plan of care as directed by the nurse; provides input for evaluation of plan of care
- Participates in Home Care interdisciplinary team meetings; utilizes telemonitoring system correctly
- Collects and communicates relevant and appropriate information to the nurse according to policy and standards of care including but not limited to: admission database, routine measurements; results of care activities; complaints/concerns of patient and ensures completion daily visit sheets
- Attends to the individual needs of patients, which may include but is not limited to assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating or other needs in keeping with the individuals' care requirements
- Inquires about patient's needs prior to leaving home; observes patients for changes in condition or behavior and promptly reports these changes to appropriate nursing personnel
- Maintains the comfort, privacy, and dignity of patients in the delivery of services to them; interacts with patients in a manner that displays warmth and promotes a caring environment
- Lifts, moves, and transports patients, using proper body mechanics or lifting devices for accident prevention
- Performs all aspects of the position in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors; utilizes established hand-off communication protocols; maintains the security of the department
- Ensures documentation is complete, accurate and timely; communicates documented information to nurse for evaluation; delivers and files physician orders as directed
- Performs additional duties and responsibilities as deemed necessary

- Ability to handle difficult situations involving patients, providers or others in a professional manner
- Maintains confidentiality in all aspects of the job

Position Requirements:
- Computer word processing skills
- Current IL HHA certification with at least one year of applicable experience
- BLS required within 30 days of hire
- Valid driver’s license and current car insurance
- Ability to work independently, prioritize and complete tasks in a timely manner
- Excellent communication and customer service skills
- Ability to communicate, including the ability to read, write, and verbally report information
- Knowledge of state, federal, accreditation and regulatory agency laws and requirements governing home healthcare
Pay Range:
Pay ranges from $15.250/hour to $20.979/hour (rate of pay is based on applicable years of experience)
Hours: Monday- Friday 8am-430pm
